Walk down any supplement aisle and the probiotic labels seem to compete on a single number: how many billions of bacteria are crammed into each capsule. It is easy to compare at a glance, but it tells you far less about how well a probiotic might actually work than the marketing suggests. To understand why, it helps to look past the label at what these microbes do once they reach the gut, and why two products boasting the same number can be worlds apart.

The billion-CFU myth

CFU stands for colony-forming units, a count of the live microbes in each dose. A bigger number feels like better value: surely more bacteria add up to a more effective product. That is exactly why so many labels lead with it.

In reality, no universal rule says a higher CFU count makes a probiotic work better. The dose that matters depends on which bacteria you are taking and what has been studied. Some well-researched probiotics show benefits at fairly modest counts, while an enormous count of an untested mixture carries no guarantee of doing anything at all. Beyond the dose a strain was actually studied at, extra billions may simply add to the price. CFU tells you quantity, not quality, and not whether that quantity is even relevant to you.

What a probiotic's name actually tells you

Probiotics are classified with a three-part name, and each part narrows things down. Take the well-known example Lactobacillus rhamnosus GG:

  • Genus, the broad family the microbe belongs to (Lactobacillus).
  • Species, a more specific group within that family (rhamnosus).
  • Strain, the exact, identifiable variant, often marked by a code or letters (GG).

The strain matters most, because the benefits of probiotics are strain-specific. Think of it like dog breeds: a sheepdog and a lapdog are both dogs, yet suited to very different jobs. Two strains of one species can likewise carry different genes, produce different compounds and behave quite differently in the body. Research is done on particular strains for particular outcomes, so evidence that one may support digestive comfort tells you little about an unrelated strain, even one with a similar-sounding name. A documented, named strain therefore means more than a genus alone, and a random high-count blend is not interchangeable with one that has genuinely been studied.

What probiotics actually do in the gut

It is easy to call probiotics "good bacteria" without explaining what that goodness consists of. The living microbes in your gut, collectively the microbiome, are not passive passengers; they are chemically and physically busy, and probiotic strains are studied for a handful of specific jobs. Not every strain does all of these, but the main mechanisms look something like this.

Crowding out less-helpful microbes

The gut is crowded, and space and food are finite. One way beneficial bacteria are thought to help is competitive exclusion, competing with less-helpful microbes for the same nutrients and footholds on the gut lining. By taking up room and resources, and sometimes producing acids that make the neighbourhood less hospitable, they make it harder for problem microbes to establish. It is less about "killing off the bad guys" than tilting a competitive balance.

Supporting the gut barrier

The lining of your intestine is only one cell thick in places, and those cells are held together by structures called tight junctions that control what passes into the bloodstream. A healthy barrier lets nutrients through while keeping bacteria and their by-products where they belong. Some strains appear to support it, encouraging a healthy mucus layer, say, or helping keep those junctions intact. The evidence is strain-specific and still developing, but the idea is that a robust barrier is part of a settled, comfortable gut.

A conversation with the immune system

A large share of the body's immune tissue sits in and around the gut, putting resident microbes in constant dialogue with immune cells. Rather than crudely "boosting" immunity, a phrase worth treating with suspicion, certain strains appear to help modulate immune signalling, nudging it toward a measured rather than over-reactive response. It is an active research area where marketing runs well ahead of the evidence, so be cautious about sweeping claims.

Producing beneficial compounds

Perhaps the most underrated thing gut bacteria do is manufacture. As they ferment the fibre we cannot digest ourselves, they release compounds such as short-chain fatty acids, which fuel the cells lining the colon and are the subject of intense research. These by-products, increasingly grouped under the term postbiotics, are one reason a probiotic's effects can outlast the microbe that produced them.

Probiotics, prebiotics and postbiotics

Three similar-sounding words get used almost interchangeably in marketing, yet they describe quite different things, and keeping them straight makes supplement claims much easier to read.

  • Probiotics are the live microbes themselves, bacteria (and sometimes yeasts) intended to confer a benefit when taken in adequate amounts.
  • Prebiotics are not alive at all. They are types of fibre and other compounds that feed the beneficial bacteria already in your gut, fertiliser rather than seeds.
  • Postbiotics are the useful by-products bacteria create as they work, such as the short-chain fatty acids above.

Pairing a probiotic with a prebiotic is sometimes called a synbiotic approach: one supplies the organisms, the other helps support them once they arrive. The logic, that bacteria need a food source to establish themselves, is well established, even if research on specific combinations is still maturing. The practical point: a probiotic does not act in isolation, and what you feed your existing microbes day to day, largely through fibre-rich food, matters at least as much as any capsule.

Getting there alive, and what the label leaves out

A probiotic can only do any of this if the bacteria reach your gut alive, and on the way they must pass through stomach acid and bile, conditions many microbes do not survive. This is why formulation and delivery matter as much as the organisms themselves: how a product is made, coated and protected influences how many bacteria arrive intact.

Storage plays a role too. Some probiotics are sensitive to heat and moisture, so a count printed at manufacture is not necessarily the count you receive by the end of shelf life, particularly if the product has sat in a hot warehouse or a sunny kitchen. Reputable makers guarantee a count through to the expiry date, not just at manufacture; the difference can be large.

The quality gap

There is also a broader quality problem worth knowing about. Probiotics are generally regulated as foods or supplements rather than medicines, so scrutiny is lighter than many shoppers assume. Independent testing has repeatedly turned up products with fewer live organisms than the label promises, and occasionally strains that are mislabelled or barely present. This does not mean probiotics do not work; it means the gap between a careful and a careless product is wide, and the label alone will not always reveal which you hold. Third-party testing, clear strain identification and an honest end-of-shelf-life count are the signs of a maker taking that gap seriously.

The "reseeding" myth: visitors as well as residents

Much probiotic marketing leans on "restoring", "reseeding" or "rebuilding" your gut, as though a course of capsules plants a permanent new garden. The reality is more nuanced.

Your gut already hosts an enormous, highly individual community of microbes, shaped over years by your genetics, diet, environment and history. Most probiotic strains taken by mouth behave less like settlers than visitors: research suggests they largely pass through, often undetectable a week or two after you stop. They tend not to permanently colonise a gut that is already fully occupied.

That is not really bad news. A microbe can still have an effect while passing through, interacting with the immune system, producing metabolites, competing with other organisms as it goes. It simply means benefits often depend on continued use rather than a one-off reset, and that no capsule overwrites your existing microbiome wholesale. "Support" is a more honest verb than "restore".

Fermented foods versus capsules

It is fair to ask how a supplement compares with fermented foods, yoghurt, kefir, sauerkraut, kimchi, miso and the like.

Both have their place, and are not really rivals. Fermented foods deliver live cultures alongside fibre and other nutrients, and the food itself may help some microbes survive the trip; they are a pleasant, low-cost daily habit. The trade-off is uncertainty: the strains and counts are usually unknown and vary from batch to batch, and some products, many shelf-stable pickles, most bread, are not live-fermented at all, or are heat-treated afterwards, which knocks the microbes out.

A capsule offers the opposite balance: a specific, named strain at a known, studied dose, with some assurance about how many organisms are still alive. That precision is what you want if you are trying to match the conditions of the research. For most people the sensible reading is "both, for different reasons": fermented foods as an enjoyable staple, and a targeted supplement when there is a specific reason to reach for one.

Who is most likely to benefit

Probiotics are marketed as close to universal, but the evidence is stronger in some situations than others, and "everyone should take one every day" is not well supported. Where the research is comparatively encouraging tends to be specific and strain-dependent, particular strains studied around digestive comfort, say, or settling the gut during and after a course of antibiotics. For a generally healthy person eating a varied, fibre-rich diet, a daily probiotic may add little; feeding the microbes you already have often does more than importing new ones.

A few sensible principles hold across the board:

  • Match the strain to the outcome you care about, using what has been studied, rather than buying on CFU count alone.
  • Give it a fair, consistent trial, often several weeks, since effects usually depend on continued use.
  • Individual responses vary, because everyone starts from a different microbiome.
  • If you are pregnant, immunocompromised, seriously unwell or managing a diagnosed condition, treat probiotics as something to discuss with a health professional rather than self-prescribe.
